Home Based Acute Care Manager
Our client is seeking a Home Based Acute Care Manager to provide strategic leadership and operational oversight for a growing home-based acute care program. This role ensures high-quality, efficient patient care delivered in the home, while driving strong cross-functional collaboration and maintaining fiscal responsibility for program operations.

This role is onsite in Portland, Oregon.

Home Based Acute Care Manager Responsibilities

  • Provide overall leadership, direction, and daily operational management for the Home Based Acute Care program to ensure safe, effective, and patient-centered care.
  • Oversee program performance, workflows, and service delivery to support consistent, high-quality clinical outcomes.
  • Maintain fiscal accountability for the department, including developing, monitoring, and managing the operational budget.
  • Foster strong partnerships with the program’s command center team, clinical staff, and internal and external service vendors to ensure seamless coordination of care.
  • Identify opportunities to enhance operational efficiency, streamline processes, and support ongoing program development.
  • Ensure compliance with policies, regulatory standards, and best practices for home-based acute care delivery.
  • Provide leadership, guidance, and support to assigned staff, promoting a collegial and collaborative work environment.

Home Based Acute Care Manager Qualifications

  • Active RN license with experience leading clinical teams or operations within acute care, home health, or a related care delivery environment.
  • Demonstrated leadership skills with the ability to manage program operations and guide multidisciplinary teams.
  • Experience with operational oversight, performance improvement, and patient-care workflow management.
  • Proven ability to manage budgets, monitor expenses, and ensure responsible resource utilization.
  • Strong interpersonal and communication skills with the ability to build effective relationships across diverse stakeholders.
  • Knowledge of care coordination processes, regulatory requirements, and best practices related to home-based or acute care services.
  • Ability to think critically, problem-solve, and adapt to evolving program needs.

Compensation: $55-$65/hr. (DOE)
