Manager, Care Management
Our client is seeking a Manager, Care Management to lead a multidisciplinary care management function within a complex acute care environment. This individual will oversee clinical and operational activities related to care coordination, utilization oversight, discharge planning, and psychosocial support services while ensuring high-quality patient outcomes, regulatory compliance, and efficient resource utilization. The ideal candidate is a collaborative healthcare leader with strong hospital-based case management experience and a passion for improving the patient care journey.

This role is on-site in Everett, Washington.

Manager, Care Management Responsibilities

  • Provide day-to-day leadership, oversight, and operational management for the Care Management department, including care coordination, case management, discharge planning, utilization review, social work services, and pre-admission planning.
  • Lead, mentor, and develop department staff while fostering a high-performing, patient-centered team culture focused on accountability, collaboration, and continuous improvement.
  • Establish, implement, and monitor department workflows, best practices, and clinical standards to support quality patient outcomes, operational excellence, and sound financial stewardship.
  • Partner closely with physicians, nursing leadership, interdisciplinary care teams, and external healthcare partners to support effective care transitions and coordinated patient care planning.
  • Serve as a key escalation point for complex patient care coordination matters involving patients, families, internal stakeholders, post-acute providers, and payer representatives.
  • Drive compliance with regulatory requirements, accreditation standards, and organizational policies related to care management and utilization practices.
  • Analyze departmental performance metrics, identify opportunities for process improvement, and implement strategies to improve efficiency, throughput, and patient experience.
  • Support resource planning, staffing optimization, and departmental goal achievement in alignment with broader organizational objectives.

Manager, Care Management Qualifications

  • Master’s Degree in Social Work from an accredited institution.
  • Active Washington Clinical Independent Social Worker (LICSW) license required.
  • 6+ years of progressive experience in case management within an acute care or hospital environment.
  • 2+ years of leadership experience managing care management, case management, or related clinical support teams, preferably within a large hospital setting.
  • Strong understanding of discharge planning, utilization management, care coordination, social work services, and healthcare reimbursement practices.
  • Demonstrated ability to lead cross-functional teams, influence stakeholders, and navigate complex patient care and operational challenges.
  • Experience developing departmental processes, implementing best practices, and using performance data to drive operational improvement.
  • Excellent communication, leadership, relationship management, and problem-solving skills.

Salary: $113K-$178K/year. (DOE)
