Manager Care Management – Social Work
Our client is seeking an experienced Manager of Care Management – Social Work to lead a multidisciplinary team responsible for care coordination, case management, and social work services within a hospital setting. This role requires a strategic and compassionate leader who can oversee complex patient care processes while ensuring operational efficiency, compliance with clinical standards, and strong collaboration across the healthcare continuum. The ideal candidate brings deep case management experience within large healthcare systems and a commitment to improving patient outcomes through coordinated care.

This role is onsite in Everett, Washington.

Manager Care Management – Social Work Responsibilities

  • Lead the daily operations of the Care Management department, overseeing case management, care coordination, social work services, utilization management, and discharge planning activities.
  • Manage, mentor, and support care management staff while fostering a collaborative, patient-centered culture across interdisciplinary teams.
  • Establish, implement, and monitor best practices and clinical standards to ensure high-quality patient care and effective care transitions.
  • Serve as a liaison between patients, families, physicians, hospital leadership, community agencies, and post-acute care providers to coordinate comprehensive care plans.
  • Monitor departmental performance, compliance, and operational workflows to support both patient outcomes and organizational financial stewardship.

Manager Care Management – Social Work Qualifications

  • Master’s Degree in Social Work (MSW) from an accredited program.
  • Active Washington State Clinical Independent Social Worker (LICSW) license required.
  • 6+ years of experience in case management within a healthcare or hospital environment.
  • 2+ years of leadership experience managing teams in care management, case management, or related clinical services.
  • Experience working in large hospital or health system environments with strong knowledge of care coordination, discharge planning, and utilization management practices.

Salary: $113K-$178K/year. (DOE)
