Care Coordinator (Remote)
Our client is seeking a compassionate and highly organized Care Coordinator who excels in outreach, communication, and guiding members through behavioral health resources. The ideal candidate is empathetic, proactive, and skilled at navigating fast-paced environments while providing exceptional support to both members and providers. This role is perfect for someone who thrives in collaborative settings, communicates with clarity, and brings a strong sense of ownership to helping individuals access the care they need.
This role is fully remote working a rotating schedule.
Care Coordinator (Remote) Responsibilities
- Conduct telephonic outreach, screenings, and post-discharge follow-ups to support access to behavioral health services.
- Triages incoming referrals and member inquiries, ensuring timely routing and coordination with internal teams and providers.
- Educate members about behavioral health conditions, treatment options, and community resources to enhance engagement and care continuity.
- Maintain accurate documentation in medical management systems and follow established workflows, SOPs, and prioritization guidelines.
- Serve as a liaison across members, providers, internal departments, and community agencies to resolve service issues and support smooth care transitions.
Care Coordinator (Remote) Qualifications
- High school diploma required; associate’s or bachelor’s degree in behavioral health or related field preferred.
- Experience in high-volume customer service, call center, healthcare administration, or data-entry environments.
- Strong communication, motivational interviewing, and member-engagement skills.
- Excellent organization, time management, and ability to multitask in a fast-paced setting.
- Intermediate proficiency with Microsoft Office; knowledge of behavioral health/medical terminology and bilingual skills preferred.
Compensation: $25-$30/hr. (DOE)
